We have loved having your daughter at GALA and we are sorry to see her go.  As a new school, we are growing and changing each day. So, we appreciate and thank you for taking the time to share your reasons for not re-enrolling your daughter at GALA. Your feedback is important to us, and we hope to use it to improve the experience of families who have chosen to stay at GALA next year. Your comments are confidential and you do not have to add your name to your submission.

This brief survey is only 16 questions and should take approximately 10 minutes to complete. You do not have to leave your name and you do not have to answer all of the questions in order to submit the form.

By the last day of school, please use the checklist below in order to "check out" of GALA.

1. Parent Assurance Form emailed or turned into the GALA office (this form assures the district that your daughter will be enrolled in a different school.)

2. Textbooks returned

3. Chromebook returned (charging cord, cover, and chromebook)

4. School-Owned items returned (library books, English class novels, musical instruments, sports uniforms or equipment, etc.)

5. Uniforms (consider donating uniforms to our used uniform sale benefitting students who can not afford uniforms).

6. Membership Toolkit (your family will be removed from our email database during the summer).
