LETTER OF COMMITMENT

CURRENT GALA PARENTS/STUDENTS
 
Every current parent/guardian is required to fill out the form below. 
SIBLINGS: Please fill out a separate form for EACH child at GALA. 

SENIORS: Graduating 12th grade seniors (Class of 2025) do not need to fill out this form.
Trouble-shooting: 

  • If the form is not presenting correctly on mobile view, try viewing on a laptop/desktop or, click HERE. 

  • Try logging into your Google account first and then navigate to this web page.

  • If you have siblings, you will see a link to fill out another form after you submit for the first sibling. Or, try opening the form on a different browser. Each student must have a separate submission.

  • Please do not guess on your Advisory teacher. If you don't know, please ask your student.

  • You will know the form has submitted correctly by checking your email for a confirmation.

FAMILIES NOT RETURNING TO GALA

  1. Fill out the commitment form above.

  2. Fill out the "Parent Assurance Form" (click below) and return to the GALA office (or email to admissions@galacademy.org) in order for your student to be released to a new school and for your transcripts to be transferred. Please turn in as soon as you know you are not returning. Public and private school deadlines are in March and April so please turn in by the end of April. Date the top of the form the last day your child will be attending GALA. For 2024-2025, the last day of school is June 10, 2025.

  3. Return all textbooks, High School sports uniforms, and Chromebook (with power cord and case) by the last day of school.

  4. Fill out our optional "Exit Survey" (click below) to let us know about your experience at GALA.

  5. Graduating Seniors (12th grade Class of 2025) do not need to fill out the Parent Assurance Form.
